Parent Waiver: I hereby certify that my child is in good health and has my permission to participate in the 2020 Hoop Gains Basketball Camp. I give the staff authorization to seek medical help and authorize all medical and hospital procedures as may be performed in the event of an emergency. Hoop Gains Basketball Camp, Directors, Coaches, Staff, Hoop Gains LLC, and Abington Friends School are not responsible for any incident, accident, medical/dental expenses or any other expense incurred as a result of my child’s participation in the camp. 

Photo/Video Release: As a parent or guardian of this child, I hereby consent to the use of photographs and video taken during the camp for promotional and marketing uses only on social media, print, and other media sources. I do this with full knowledge and consent and waive all claims for compensation for use, or for damages.
